Denial Code (Reason Code)

A numeric or alphanumeric code on your EOB or denial letter that identifies why your claim was denied. Common codes include CO-4 (incorrect procedure code), CO-11 (diagnosis inconsistent with procedure), and PR-96 (non-covered charge).
