The Health Insurance System in Cali

Healthcare in Cali operates primarily through the Sistema General de Seguridad Social en Salud (SGSSS), Colombia's mandatory health insurance framework:

EPS in Cali: The main Entidades Promotoras de Salud (health insurers) operating in the city include:

  • Coomeva EPS: One of Colombia's largest EPSs, historically with a strong presence in Valle del Cauca. Coomeva has faced serious financial difficulties and regulatory intervention in recent years, which has created coverage disruptions for its affiliates.
  • Nueva EPS: The successor to the old ISS (Instituto de Seguros Sociales), Nueva EPS serves a large portion of formal workers in Cali.
  • Salud Total, Sura, Sanitas: Additional EPSs with operations in the city.

Subsidized Regime: Low-income Caleños receive state-subsidized coverage managed through ADRES (Administradora de los Recursos del Sistema General de Seguridad Social en Salud).

Private healthcare: Cali has a robust private sector, including the UNIMEC network (Unidad Médica Especializada) and Clínica Imbanaco, which offer voluntary health plans and specialized care beyond what EPSs typically provide.

Why Claims Get Denied in Cali

Insurance denials in Cali follow patterns seen throughout Colombia:

  • PBS exclusions: The EPS claims the service isn't covered under the Plan de Beneficios en Salud. Courts have repeatedly ruled that medically necessary services must be provided regardless.
  • Coomeva-specific issues: Given Coomeva's financial instability and government intervention, its affiliates have experienced higher Denial Rates by Insurer (2026)" class="auto-link">denial rates and delays in care authorizations.
  • Prior Authorization Denied: How to Appeal" class="auto-link">Prior authorization failures: Services requiring prior approval (autorización previa) are frequently denied due to administrative backlogs.
  • Out-of-network claims: The EPS argues the provider used is outside its preferred network.
  • Pre-existing condition disputes: Private insurers may retroactively claim undisclosed conditions to deny claims.

Colombia's constitution and health laws provide among the strongest patient protections in Latin America:

Law 1751 of 2015 — Ley Estatutaria de Salud: Health is a fundamental right. Any denial of medically necessary care, even if not explicitly listed in the PBS, can be challenged as a violation of this law.

Resolución 3512 de 2019: Establishes the right of patients to receive a written explanation within 24 hours when a service is denied.

Time-sensitive: appeal deadlines are real.
Most insurers require appeals within 30–180 days of denial. After that, you lose your right to contest. Start your free appeal now →

Acción de Tutela: Colombia's most powerful tool for patients — a constitutional emergency petition you can file at any courthouse for free. Judges typically rule within 10 days and routinely order EPSs to provide denied services immediately.

How to Appeal a Denial in Cali

Step 1: Get the Denial in Writing

Demand a written denial (negativa) from your EPS. If Coomeva, Nueva EPS, or another insurer denies you verbally or delays, document the date and the name of the person you spoke with, and immediately send a written request for the formal denial letter.

Step 2: File a PQRD with Your EPS

Submit a formal Petición, Queja, Reclamo o Denuncia (PQRD) to your EPS. For Nueva EPS, offices are located throughout Cali, including in the San Fernando and Granada neighborhoods. For Coomeva, you can submit online at coomeva.com.co or visit their main Cali office. Your EPS has 15 business days to respond (or 10 days for urgent health situations).

Step 3: Escalate to Superintendencia Nacional de Salud (Supersalud)

Supersalud is the national health insurance regulator and has authority over all EPSs operating in Cali. File a complaint online at supersalud.gov.co or call their national helpline 018000-513700. Given Coomeva's special regulatory situation, Supersalud has additional oversight mechanisms for its affiliates.

Step 4: File an Acción de Tutela

For urgent or medically necessary care, file an acción de tutela at any Cali courthouse (Juzgado Civil del Circuito or Juzgado Administrativo). Bring your denial letter and doctor's medical justification. Courts in Valle del Cauca have a strong track record of ruling against EPSs that deny essential care.

Step 5: Contact the Defensoría del Pueblo — Valle del Cauca

The Defensoría del Pueblo Regional Valle del Cauca, located on Calle 13 in Cali, offers free legal support to patients appealing denials. They can assist with drafting tutelas and navigating Supersalud complaints.

Private Insurance Complaints in Cali

If your claim was denied by a voluntary private insurer (not an EPS), the relevant regulator is the Superintendencia Financiera de Colombia (SFC), which oversees private health insurers and insurance companies. File complaints at superfinanciera.gov.co or visit their regional representative office in Cali.

Coomeva Affiliates: Special Considerations

If you were affiliated with Coomeva EPS, you may have been reassigned to another EPS as part of the government's intervention process. Check with the Ministerio de Salud (minsalud.gov.co) to confirm your current EPS status, as some Coomeva denials may require escalation through the liquidation process rather than the standard PQRD channel.
