Expat Health Insurance Denied in Dhaka? How to Appeal in Bangladesh

Health insurance claim denied in Dhaka — through MetLife Bangladesh, Green Delta Insurance, or another insurer at Apollo Hospitals Dhaka? Here's how to appeal under IDRA oversight.

Expat Health Insurance Denied in Dhaka? How to Appeal in Bangladesh

Dhaka is Bangladesh's capital and its economic heart, home to the largest expat communities in the country — primarily NGO workers, garment industry professionals, and diplomatic personnel. Private health insurance from companies like MetLife Bangladesh and Green Delta Insurance provides coverage for both high-income Bangladeshis and expats at facilities like Apollo Hospitals Dhaka and United Hospital.

When a claim is denied, understanding Bangladesh's insurance regulatory framework gives you real options for appeal.

The Dhaka Health Insurance Landscape

Bangladesh does not have a comprehensive mandatory national health insurance scheme for the general population, though a National Health Insurance (NHI) program and the Health Promotion Foundation have been under development. Social protection for formal workers falls under the Bangladesh Labour Act for certain employee benefits, but health insurance is primarily a private-sector product.

Major private life and health insurers in Dhaka:

  • MetLife Bangladesh (American Life Insurance Company) — one of the largest and most reputable foreign-linked life and health insurers; strong in group corporate plans
  • Green Delta Insurance Company — leading non-life insurer; offers group health, personal accident, and medical expense insurance
  • Pragati Life Insurance — large domestic life insurer with health riders
  • Guardian Life Insurance — growing domestic life and health insurer
  • National Life Insurance Company — state-connected major domestic insurer
  • Rupali Insurance — non-life insurer with health products
  • Reliance Insurance Limited — non-life health and accident products

International plans used by NGO workers and diplomats include Cigna Global, Allianz Care, and the UN's emergency health plans.

Key hospitals where claims arise:

  • Apollo Hospitals Dhaka — premier private hospital; widely accepted by domestic and international plans
  • United Hospital — high-quality private hospital in Gulshan
  • Square Hospital — major private hospital in Panthapath
  • Evercare Hospital Dhaka (formerly Apollo Hospitals, Bashundhara)
  • Labaid Hospital
  • Dhaka Medical College Hospital — public tertiary facility

Why Claims Are Denied in Dhaka

Domestic insurer denials (MetLife BD, Green Delta):

  • Pre-existing conditions — group health plans often exclude conditions diagnosed before the enrollment date; this is a major source of disputes for employees who enroll with chronic illnesses
  • Non-panel hospital — treatment at a facility not on the insurer's approved network without pre-authorization
  • Documentation deficiencies — missing original hospital invoices, discharge summary in wrong format, or attending physician report not provided
  • Annual benefit limit reached — common for serious conditions requiring extended treatment
  • Waiting period violations — although group health plans usually have shorter waiting periods than individual plans, disputes can still arise
  • Pre-authorization not obtained for elective procedures

Green Delta (non-life health insurance) denials:

  • Procedure not covered under the specific policy type purchased (accident vs. illness distinction)
  • Claim submitted outside the 30–60 day filing window
  • Sub-limit for specific treatment categories exceeded

International plan denials:

  • Bangladesh country-specific coverage sub-limits
  • Medical evacuation necessity disputed
  • Pre-authorization not obtained before treatment

Step 1 — Get the Denial in Writing

Under Bangladesh's Insurance Act 2010 and IDRA regulations, insurers must communicate claim decisions in writing with stated grounds. Request a formal written denial letter specifying the exact policy clause or condition cited.

Step 2 — Internal Appeal

For MetLife Bangladesh:

  1. MetLife Bangladesh has a formal customer service and claims dispute process. Contact MetLife BD's Claims Department at their Dhaka head office (MetLife House, 166 Gulshan Avenue, Gulshan 1, Dhaka 1212).
  2. Submit a written appeal with complete medical documentation: discharge summary from Apollo Hospitals Dhaka or your treating facility, physician letter of medical necessity, all diagnostic reports, and itemized invoice.
  3. MetLife Bangladesh, as a regulated international insurer, has higher compliance standards and generally responds to well-documented appeals.

For Green Delta Insurance:

  1. Submit your written appeal to Green Delta's Claims Department at their Dilkusha office (Green Delta AIMS Tower, 51-52 Mohakhali Commercial Area).
  2. Include all supporting documentation and a clear written statement disputing the denial reason.
  3. Green Delta has an internal review process for disputed claims.

For other domestic insurers:

  1. Submit appeals in writing to the respective Claims or Customer Service Department.
  2. Include supporting medical documentation and a clear statement of the grounds for your appeal.

Step 3 — Escalate to IDRA

The Insurance Development and Regulatory Authority (IDRA) is Bangladesh's insurance regulator, established under the Insurance Development and Regulatory Authority Act 2010:

  • Address: Jiban Bima Tower (17th Floor), 10 Dilkusha Commercial Area, Dhaka 1000
  • Phone: +880-2-9562580 to 82
  • Email: info@idra.org.bd
  • Website: www.idra.org.bd

IDRA has authority over all licensed insurers in Bangladesh and can:

  • Investigate complaints about wrongful claim denials
  • Compel insurer responses to unresolved disputes
  • Take regulatory action against insurers that violate the Insurance Act 2010
  • Mediate between policyholders and insurers

File your IDRA complaint with copies of your policy, denial letters, appeal correspondence, and full medical records.

Step 4 — Bangladesh Bank and Consumer Rights Commission

For disputes involving banks that sold insurance products (bancassurance), you may also file with Bangladesh Bank as the banking regulator.

The National Consumer Rights Protection Directorate (DNCRP) under the Ministry of Commerce can also handle insurance consumer complaints under the Consumer Rights Protection Act 2009.

Building a Strong Appeal in Dhaka

Effective appeals for Dhaka claims include:

  • Physician letter from Apollo Hospitals Dhaka, United Hospital, or your treating facility specifically addressing medical necessity
  • Complete hospital records — admission notes, treatment plan, diagnostic results, discharge summary
  • Itemized hospital invoice with procedure codes
  • Evidence of premium payment continuity (especially important for group plans where employer contributions are involved)
  • Specific policy language showing the benefit clause entitling you to coverage
