Humana Ozempic or GLP-1 Denied? Weight Loss Drug Appeal Guide

Humana denied Ozempic, Wegovy, or another GLP-1? Learn about formulary tiers, step therapy, Medicare Part D restrictions, and how to file a coverage exception.

GLP-1 receptor agonists — including semaglutide (Ozempic, Wegovy), tirzepatide (Mounjaro, Zepbound), and liraglutide (Victoza, Saxenda) — are among the most prescribed and most denied medications in American healthcare today. If Humana denied your GLP-1 prescription, the denial reason and your path to appeal depend entirely on your plan type and the medication's intended use. This guide breaks it down clearly.

The Crucial Distinction: Diabetes vs. Weight Loss

GLP-1 receptor agonists are FDA-approved for two distinct indications:

  1. Type 2 diabetes management: Ozempic (semaglutide), Mounjaro (tirzepatide), Victoza (liraglutide), Trulicity (dulaglutide), and others are FDA-approved to improve glycemic control in adults with type 2 diabetes.

  2. Chronic weight management (obesity): Wegovy (semaglutide), Zepbound (tirzepatide), and Saxenda (liraglutide) are FDA-approved specifically for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27) with a weight-related comorbidity.

Humana covers these medications very differently depending on the indication. Understanding which brand was prescribed and for which indication is the first step in your appeal.

Medicare Part D: Weight Loss GLP-1s Are Not Covered

If you have a Humana Medicare Part D prescription drug plan or a Humana Medicare Advantage plan with Part D coverage, you face a hard barrier: Medicare Part D is currently prohibited by federal law from covering weight loss drugs. This is not a Humana policy decision — it is a statutory exclusion in the Social Security Act (42 U.S.C. § 1395w-102(e)).

This means:

  • Wegovy (semaglutide for weight loss) is NOT covered by any Medicare Part D plan
  • Zepbound (tirzepatide for weight loss) is NOT covered by any Medicare Part D plan
  • Saxenda (liraglutide for weight loss) is NOT covered by any Medicare Part D plan

However, if you have type 2 diabetes AND your physician prescribes Ozempic or Mounjaro for diabetes management (not weight loss), these ARE covered under Medicare Part D if they appear on the formulary. The brand name matters: Ozempic is FDA-approved for diabetes; Wegovy is FDA-approved for weight loss — even though both contain semaglutide.

For Medicare members denied a GLP-1 for weight loss, the only current path is to: (1) confirm the prescription is for diabetes if that is applicable, or (2) participate in clinical trials or advocacy for the pending Medicare weight loss drug coverage expansion.

Commercial Humana Plans: Formulary and Step Therapy

For Humana employer-sponsored or individual/family commercial plans, GLP-1 coverage depends on your specific plan's formulary. Key points:

Formulary tier placement: GLP-1 drugs are typically placed on Tier 3 (preferred brand) or Tier 4 (non-preferred brand) on Humana's commercial formularies, resulting in high cost-sharing. Some plans exclude GLP-1s for weight loss entirely.

Step therapy requirements: Humana commercial plans frequently require patients to try and fail on less expensive alternatives before GLP-1s are authorized. For diabetes, this typically means documenting that metformin and at least one other oral agent were tried. For weight loss, step therapy may require documented participation in a structured weight loss program or prior use of another weight loss medication.

Prior Authorization Denied: How to Appeal" class="auto-link">Prior authorization: Most GLP-1 prescriptions require prior authorization from Humana, regardless of formulary tier.

Weight loss benefit exclusion: Many employer-sponsored plans explicitly exclude weight loss drugs from coverage as a plan design choice. If your plan has this exclusion, it applies to all weight loss medications, not just GLP-1s.

How to File a GLP-1 Coverage Exception with Humana

If Humana denied your GLP-1 due to formulary status, step therapy, or prior authorization, you have the right to request a formulary exception or coverage exception. Here is the process:

Step 1: Identify the denial type. Is this a formulary exclusion, a step therapy denial, a prior authorization denial, or a plan exclusion for weight loss drugs? Each requires a different approach.

Step 2: Have your prescriber submit a prior authorization with supporting documentation. Humana's PA requirements for GLP-1s typically include:

  • Diagnosis (Type 2 diabetes with HbA1c value, or obesity with BMI and comorbidity documentation)
  • Documentation of prior medications tried (with dates, doses, and reason for discontinuation)
  • Current medications
  • Cardiovascular risk factors or cardiovascular disease history (semaglutide and tirzepatide have proven cardiovascular benefit in high-risk patients)

Step 3: Request a formulary exception if the drug is not on Humana's formulary. A formulary exception requires your prescriber to document that the formulary alternatives are medically inappropriate or contraindicated for you specifically.

Step 4: Request a step therapy exception. If Humana requires you to try other medications first, your prescriber can document that:

  • You already tried the required medications and they were inadequate, ineffective, or caused adverse effects
  • A contraindication prevents you from taking the required step therapy drugs

Step 5: File a formal internal appeal if the exception request is denied. Include the prescriber letter, clinical documentation, and relevant peer-reviewed literature supporting the medical necessity of the specific GLP-1.

Step 6: Request External Independent Review: Complete Guide" class="auto-link">external review if the internal appeal is denied. For commercial plans, an IRO will conduct an independent medical review.

Evidence That Strengthens a GLP-1 Appeal

  • HbA1c values showing inadequate glycemic control on current regimen (for diabetes indication)
  • Documented cardiovascular disease or high cardiovascular risk (Ozempic and Mounjaro have FDA-approved cardiovascular indications)
  • BMI measurement with documented comorbidities (hypertension, sleep apnea, dyslipidemia) for weight loss indication
  • Records of prior weight loss attempts including structured programs, prior medications, and outcomes
  • Documentation of contraindications to formulary alternatives
  • Peer-reviewed studies or FDA labeling supporting the specific GLP-1 for your condition
