Most insurance plans require documented failure of conventional DMARDs (disease-modifying antirheumatic drugs) such as methotrexate, sulfasalazine, hydroxychloroquine, or leflunomide before approving a biologic like adalimumab for RA or PsA. For IBD, insurers typically want evidence of corticosteroid dependence and failure of immunomodulators like 6-mercaptopurine or azathioprine.
