Its Enhanced IncomeShield (previously OrangeShield) Integrated Shield Plan is one of the most widely held health insurance products in the country, covering millions of Singaporeans alongside MediShield Life. Why Income Insurance Denies Claims

Enhanced IncomeShield / OrangeShield Denials

  • Pre-existing condition exclusion: Income Insurance may deny a claim if it determines that the condition was present — or that symptoms existed — before you took out the policy.
  • Non-disclosure: If income from prior medical consultations or conditions was not declared, Income Insurance may cite non-disclosure as grounds for denial or policy rescission.
  • Ward class mismatch: Enhanced IncomeShield has multiple tiers (Standard, Preferred, Advantage). Hospitalisation at a class above your covered tier will not be fully covered.
  • Rider dependency: The Assist Rider reduces or eliminates co-payments and deductibles, but only for eligible claims under the main policy. If the main claim is denied, the rider claim is also denied.
  • Out-of-panel treatment: Income Insurance operates a panel of preferred specialists and hospitals. Claims arising from treatment by non-panel providers may be subject to restrictions.
  • Late claims submission: Most plans require submission within 30–90 days of discharge or treatment.

Life and Critical Illness Claims

  • CI definition disputes: Income Insurance's CI policies follow the LIA Singapore standardised definitions. Denials often cite that the diagnosis does not meet the specific stage or severity threshold defined in the policy.
  • Death claim exclusions: Life insurance death claims may be denied for suicide within the exclusion period (typically 1 year), death under undisclosed high-risk activities, or disputes over the cause of death.

Step 1: Request the Formal Denial

Ask Income Insurance for a written decision with the specific policy clause cited. If the denial was communicated verbally, follow up in writing to create a paper trail.

Step 2: File an Internal Appeal

Income Insurance has a formal complaints process. Submit a written appeal to their Customer Care team, including:

Time-sensitive: appeal deadlines are real.
Most insurers require appeals within 30–180 days of denial. After that, you lose your right to contest. Start your free appeal now →
  • Policy number and claim reference
  • A point-by-point rebuttal with supporting evidence
  • Specialist letters, hospital records, discharge summaries
  • Any correspondence that supports your position (e.g., pre-authorisation emails, agent advice in writing)

Under MAS guidelines, Income Insurance must acknowledge your complaint within 5 business days and resolve it within 21 business days (up to 45 for complex cases).

Step 3: Escalate to FIDReC

If the internal appeal is unsuccessful or ignored, file with FIDReC (Financial Industry Disputes Resolution Centre). FIDReC is:

  • Free for consumers (no filing fee)
  • Independent of Income Insurance
  • Binding on the insurer for awards up to S$100,000

FIDReC adjudicates both contractual and equitable grounds. If an Income Insurance agent gave you misleading advice at point of sale, FIDReC can take this into account.

ISP-Specific Rules for Enhanced IncomeShield

Enhanced IncomeShield is an Integrated Shield Plan regulated by both MAS and the Ministry of Health (MOH). Important considerations:

  • MediSave for premiums: You can use CPF MediSave to pay for the basic Enhanced IncomeShield premium up to the applicable Additional Withdrawal Limit. The rider premium must be paid in cash.
  • IDAC: If the dispute relates specifically to the ISP component and its alignment with MediShield Life benefits, the Integrated Development and Assessment Committee (IDAC) under LIA Singapore can review the application.
  • MOH-approved list: Certain treatments and procedures are covered only if they appear on the MOH's Surgical Procedures Table or are certified by a MOH-recognised specialist.

Tips for a Strong Appeal

  • Obtain a specialist's letter confirming medical necessity
  • Gather evidence that the condition arose after your policy start date if pre-existing conditions are disputed
  • If non-disclosure is alleged, demonstrate that the information was not material to Income Insurance's underwriting decision
  • Reference the LIA's Best Practices Guide on Claims Handling in your appeal
